How clear aligners work

You wear a sequence of trays, each slightly different from the last. Every tray moves teeth a small amount, and you change to the next one on a set schedule until the sequence is complete.

Trays are removed for eating, drinking anything other than water, and cleaning your teeth. They need to be worn 20 to 22 hours a day to work as planned.

Where aligners work well

Mild to moderate crowding, small gaps, and mild relapse after previous orthodontic treatment.

Where fixed braces are usually the better answer

Significant crowding or rotation, substantial bite correction, cases needing teeth moved bodily rather than tipped, and patients who realistically will not wear trays 20+ hours a day.

That last one matters more than people expect. Aligners only move teeth while they are in your mouth. Braces work continuously whether or not you remember them. A teenager who leaves trays in a school bag will get a worse result than they would have from braces, and that is worth being honest about before spending more money.

Invisalign and other systems

Invisalign is the best-known clear aligner brand, and many people use the name to mean clear aligners generally. Several systems exist and work on the same principle.

What treatment involves

1. Assessment. Examination, X-rays and photographs to determine whether aligners suit your case. If they will not give a good result, we will say so.

2. Planning. We use both conventional impressions and an intraoral scanner, depending on the case, to plan the sequence of movements.

3. Wearing the trays. Each tray is worn for a set period before moving to the next. Some cases need small tooth-coloured attachments bonded to certain teeth to allow particular movements.

4. Reviews. Periodic check appointments to confirm teeth are tracking as planned.

5. Retainers. As with braces, a retainer is needed afterwards to hold the result.

Frequently asked questions

How long does treatment take? It depends on how much movement is needed. Simple cases finish considerably faster than complex ones. Your dentist will estimate after the assessment.

Do aligners hurt? Each new tray feels tight for a day or two as teeth begin moving. That is the treatment working. Most people find it comparable to, or milder than, an adjustment with braces.

Can I take them out for a wedding or an interview? Yes, briefly. That flexibility is one of the main advantages. Consistently falling short of the daily wear time will slow treatment or compromise the result.

Can I eat with them in? No. Remove them to eat and to drink anything other than water. Hot drinks can warp the plastic, and sugary drinks get trapped against your teeth.

Are clear aligners the same as clear retainers? No. Aligners move teeth; retainers hold them where they already are. They look similar and do different jobs.

Can I get aligners if I had braces years ago and my teeth moved back? Often yes. Mild relapse is one of the cases aligners handle well. An assessment will confirm.

Will people notice? They are far less visible than braces, though not completely invisible up close, particularly if attachments are needed.
