Klinik Pergigian Genting assesses and removes wisdom teeth at our clinic on Jalan Genting Kelang, Setapak. Not every wisdom tooth needs removing. We take an X-ray and explain what it shows before recommending anything. Complex surgical cases are handled by Dr Reshnu Suri, a visiting dentist who attends when a case requires her.

When a wisdom tooth needs removing

Removal is usually recommended where there is a demonstrable problem:

Recurrent pericoronitis - inflammation of the gum flap over a partly erupted tooth, where food and bacteria become trapped. It tends to recur.

Damage to the tooth in front - an angled wisdom tooth pressing into the second molar can cause decay or resorption on a tooth that would otherwise be healthy.

Decay in the wisdom tooth itself - its position at the very back makes it hard to clean and hard to restore.

Cyst formation around an unerupted tooth.

Repeated infection or pain that has not settled with other treatment.

When it does not need removing

A wisdom tooth that has erupted fully, sits in a reasonable position, causes no symptoms and can be cleaned properly does not automatically need extracting. Removing healthy, symptom-free wisdom teeth purely as a precaution is no longer generally recommended.

If yours are not causing problems, we may suggest monitoring them at your regular check-ups instead. We will tell you what would change that.

Why an X-ray comes first

An X-ray shows what a visual examination cannot: the shape and angle of the roots, how much bone covers the tooth, and how close the roots sit to the nerve running through the lower jaw.

That determines whether removal is a simple extraction or requires minor surgery, and it is what allows your dentist to explain the specific risks in your case before you decide.

Simple extraction and surgical removal

Simple extraction. If the tooth has erupted fully and can be gripped, it is removed like any other tooth under local anaesthetic.

Surgical removal. If the tooth is impacted in gum or bone, the dentist opens a small flap of gum, sometimes removes a little bone, and often divides the tooth into sections so it can come out through a smaller opening. Stitches are usually placed.

Both are done under local anaesthetic. You are awake throughout and should feel pressure and movement, but not pain.

Sedation is not offered at the clinic. If you are anxious about the procedure, tell us at the assessment - knowing what happens at each stage helps most people, and we will work at your pace.

Recovery

Days one to three. Swelling typically peaks around 48 to 72 hours, then begins to subside. A wrapped cold compress on the cheek helps on the first day.

The first week. Swelling and discomfort ease steadily. Most people are back to normal routine within a few days.

Protect the clot. The blood clot in the socket is what protects the wound:

  • Do not smoke - it delays healing and raises the risk of dry socket
  • Do not use a straw - suction can dislodge the clot
  • Do not rinse forcefully for the first 24 hours
  • Avoid hard, very hot and spicy food for the first few days

Dry socket happens when the clot is lost before the wound has healed, causing strong pain a few days after the extraction. If your pain is getting worse after day three rather than better, contact the clinic.

How much time off?

Most people take one to two days for a simple extraction. Surgical cases may need longer, depending on the position of the tooth and your type of work.

Your dentist can give a more accurate estimate once they have seen your X-ray.

Frequently asked questions

Does wisdom tooth removal hurt? You should not feel pain during the procedure, as local anaesthetic is used. You will feel pressure and movement. Discomfort once the anaesthetic wears off is normal and manageable with painkillers.

Should all four come out at once? Not necessarily. It depends on the condition of each tooth and your own preference. Some patients prefer one side at a time so they can chew on the other.

Do wisdom teeth cause crowding of the front teeth? The relationship between wisdom teeth and front-tooth crowding remains debated among dental professionals. Wisdom teeth are not considered a sole cause.

Can I go to work the next day? Often yes for a simple extraction. Surgical removal may need a day or two more.

I am a student nearby. Can I come without an appointment? Yes. Klinik Pergigian Genting accepts walk-in patients during opening hours. The TAR UMT campus is on the same road as the clinic.

What if I leave it? An asymptomatic wisdom tooth may cause no trouble for years. One that is already causing recurrent infection or damaging the tooth in front will generally get worse, and treatment becomes more complicated. The X-ray tells us which situation you are in.
